The library has received supplies of:​

  • State forms: Form 1, Form 1-NPR, schedules AD, SB, M & WD, and Homestead booklet.
  • State instructions.

Most forms and instructions can be requested directly from the IRS or state department of revenue by following the links below.

In a hurry? The library can print most forms and instructions at our usual printing rate of $.15 / page

If the library is out of a particular form or does not carry a form that you need, copies can usually be printed by staff at the front desk at the current printing rate. Certain forms, including many beginning with 1099, cannot be printed and must instead be ordered directly from the IRS.

Help With Filing Your Taxes
Library staff will be happy to assist you in locating specific tax forms, printing out forms and publications which are not otherwise available at the library, and directing you to sources of additional help. Because library staff are not trained tax professionals, they may not assist you with deciding which forms you need or with filling out your return. For more help, please contact the IRS or the Wisconsin Department of Revenue; you can also make use of the Volunteer Income Tax Assistance program (VITA).
